На сегодняшний день в мире известно более 150 методов хирургического лечения вальгусной деформации большого пальца стопы (по Ревердену, Ювару, Шеде, Мейо и т.д.). Основной принцип оперативного лечения вальгусной деформации большого пальца стопы состоит в том, что проводится иссечение верхушки или края первого плюснефалангового сустава (той самой шишечки), а также удаление и сопоставление мягких тканей и костей с целью уменьшить боль и восстановить нормальную конфигурацию сустава.
Среди существующих операций, при этом, можно выделить физиологичные, то есть те, которые нормализуют своды стопы, оси костей и соотношения в суставах, другие же операции чисто механически устраняют существующую деформацию, не устраняя причины ее развития.
При тяжелой деформации плюснефалангового сустава используются методы фиксации его с помощью различных материалов: проволоки, винтов, швов или пластин. Однако полной гарантии на успех хирургического вмешательства при этой деформации не существует.
Операция по коррекции вальгусной деформации большого пальца стопы обычно проводится под местной анестезией. Длительность операции зависит от ее сложности и выбранного метода и может занимать около 1 часа и больше.
Как уже было сказано, существует довольно много типов оперативного вмешательства при вальгусной деформации большого пальца стопы. Перечислим их:
- Иссечение части головки первой плюсневой кости, то есть того отдела кости, за счет которого и формируется пресловутая шишечки. Эта операция называется экзостэктомия, то есть иссечение костного нароста. Это наиболее частое оперативное вмешательство по поводу вальгусной деформации большого пальца стопы.
- Восстановление и сопоставление связок вокруг первого плюснефалангового сустава (места соединения большого пальца стопы с плюсневой костью).
- Остеотомия. Эта операция заключается в удалении небольшого фрагмента плюсневой кости или фаланги пальца. Это так же одна из часто выполняемых операций по поводу вальгусной деформации большого пальца стопы. Впервые данная операция для коррекции вальгусного смещения большого пальца стопы была выполнена в 1925 году. Остеотомия может затрагивать либо плюсневую кость, либо проксимальную фалангу большого пальца, либо обе эти кости, а также клиновидную кость (кость, которая соединяется с первой плюсневой костью). Принцип остеотомии заключается в том, что кость вначале рассекается, а затем костные отломки фиксируются в нужном положении, чтобы устранить смещение большого пальца и предупредить дальнейшее развитие болезни.
- Иссечение головки плюсневой кости, в том месте, где она, соединяется с фалангой большого пальца, образуя плюснефаланговый сустав. Эта операция резекционная артропластика.
- Создание неподвижности в плюснефаланговом суставе - артродез сустава большого пальца. Данная операция направлена на разрушение сустава (сустава подразумевает подвижность, а здесь создается неподвижность). Артродез обеспечивает стабильность в суставе и предупреждает смещение большого пальца.
- Пластика плюснефалангового сустава, то есть замена сустава синтетическими имплантатами.
