Вальгусное смещение большого пальца стопы обычно сочетается с такой патологией, как поперечное плоскостопие, которое является одним из способствующих факторов данного заболевания. В происхождении этого плоскостопия, а следовательно и вальгусной деформации большого пальца, играет роль ослабление связочно-сухожильного аппарата стопы.
Именно поэтому одним из методов коррекции вальгусной деформации связан с укреплением сухожильного аппарата стопы. Обычно эта операция заключается в том, что сухожилие мышцы длинного сгибателя большого пальца стопы перемещается более медиально, то есть кнутри. В норме это сухожилие прикрепляется к дистальной (ногтевой) фаланге большого пальца стопы.
При медиальном смещении оно подшивается более кнутри, чем обеспечивается медиальное смещение большого пальца и устраняется вальгусное смешение.
Кроме того, эта операция помогает восстановить нормальный свод стопы и предупреждает дальнейшее развитие этой патологии. Вместе с пластикой сухожилия обычно может выполняться иссечение костной «шишечки».
Кроме сухожилия сгибателя большого пальца стопы проводятся операции и на дургих сухожилиях: сухожилии задней большеберцовой мышцы, которе проходит под внутренней лодыжкой и прикрепляется к бугристости ладьевидной и первой клиновидной кости, а также тонкими пучками ко II и III клиновидным, II, III и IV плюсневым костям. Эта мышца сдерживает натиск головки таранной кости, подтягивая ей навстречу ладьевидную кость, к которой прикрепляется передняя большеберцовая мышца, благодаря чему формируется продольный свод стопы.
Сухожильная пластика является одним из важных методов коррекции вальгусной деформации большого пальца стопы, так как эта операция позволяет не просто устранить проявление заболевания, а устранить причину – патологию свода стопы. Таким образом, этот вид операций является более физиологичным.
